Business Matters
Ephesians Life Ministries relies on client fees, insurance payments, and gifts and donations to support its work. Clients should discuss questions regarding fees at the time of their initial intake.
Fees
Ephesians fees for services are as follows:
Initial Intake Evaluation Session - $150.00
45-50 minute Therapy Session after the initial intake evaluation session - $100.00
Group Therapy - $30- $40 per session depending on the level of therapy.
Fees for Psychological and Educational Testing, for the interpretation of test results, and for the preparation of written reports are determined by the amount of time and expense involved in each case. This should be discussed with the therapist before testing begins.
Charge for Missed Appointments: A fee of $75 will be charged for appointments missed when cancellation is not made by twenty-four hours in advance. This charge is waived in an emergency.
Fees for educational seminars are stated in the announcements of the program.
Fee for a returned check is $30.
Insurance
Ephesians is able to accept a variety of types of health insurance. At the time of intake, we will ask for your insurance information and will verify benefits. Your counselor will collect your co-pay and we will bill the insurance company for the balance. Please note that you may be responsible for a deductible and that insurance will not pay for missed appointments.
Sliding Scale
Clients without mental health insurance and those who qualify as having low incomes are eligible for reduced fees on a sliding scale based on their income.
All clients who pay out of pocket and pay less than $100 must document need by completing a financial statement and providing an IRS form 1040 or pay stub.
